MyLeon™ is an Internet application that allows a patient to have secure web-based access to matters relating to the patient's health care from Leon Medical Centers, including access to a portion of his or her electronic medical record. It also provides a patient with a convenient method for communicating with Leon Medical Centers for non-urgent messages. Without limiting the generality of the foregoing, MyLeon™ provides the following services to patients through its web-based portal:

  • Requesting and scheduling an appointment, including requesting a change in a previously scheduled appointment;
  • Accessing and reviewing selected portions of your electronic medical record such as medical histories, medications, recent visits, etc.;
  • Receiving selected laboratory diagnostic and procedure results;
  • Communicating with Leon Medical Centers physicians and other healthcare employees regarding healthcare matters;
  • Requesting prescription refills and renewals; and
  • Linking to other web-based clinical information sites.

Terms and Conditions Acceptance of the following terms and conditions is required to access MyLeon™:

  • You understand and agree that MyLeon™ is offered by Leon Medical Centers as a convenience and courtesy to its patients. Leon Medical Centers reserves the right to terminate MyLeon™ at any time for any reason. In addition, access may not always otherwise be available (for example, in the event of system maintenance or failure). Your use of the service is optional and will not affect your status as a patient of Leon Medical Centers.
  • Once you enroll in MyLeon™, you may receive emails regarding physician messages, test results, appointment reminders, information about events at the Centers, Leon Medical Centers’ newsletter, and other communications from Leon Medical Centers through the web-based portal. In such cases, you will not likely receive duplicative written information.
  • Leon Medical Centers makes no representation or warranty as to the accuracy of the health information provided through MyLeon™, including information linked to other websites, or its suitability for use for your health care.
  • Your [login name and password] are known only by you and whoever you tell. It is your responsibility to keep your health information provided through MyLeon™ confidential and private. For that reason, you should consider changing your password on a regular basis. Any health information provided through MyLeon™ that you share with others, intentionally or unintentionally, is your responsibility.
  • You understand and agree that your messages sent through MyLeon™ may be received by Leon Medical Centers personnel other than your physician. For that reason, you may not want to use MyLeon™ for messages that are very sensitive or intended only for your physician.
  • It may take Leon Medical Centers several days to respond to a message sent through MyLeon™. Do not use MyLeon™ to send messages in emergency situations or under circumstances otherwise requiring immediate attention.
  • You may opt-out from MyLeon™ at any time by calling Leon Medical Centers Customer Service at 305-642-5366.
  • If you disenroll from Leon Medical Centers Health Plans, such that you are no longer a patient of Leon Medical Centers, you will continue to have view only access to your MyLeon™ account solely for purposes of accessing your electronic medical record and no additional information will be uploaded to your record.
  • If Leon Medical Centers determines, in its sole discretion that you have violated these terms and conditions or otherwise abused the use of MyLeon™, Leon Medical Centers can discontinue your access and use, in which case you will be notified.   • Leon Medical Centers provides the following standard-rate interactive SMS program. This campaign will be used to send opted-in subscribers messages, including appointment notifications and reminders, medication refill notifications, health maintenance reminders, and other messages from our patient portal, MyLeon. This program works with the following carriers: AT&T, Verizon Wireless, Metro PCS and T-Mobile, Msg & Data rates may apply. Carriers are not liable for delayed or undelivered messages.

  • Note that opting-out (or canceling subscription) of receiving SMS text messages will unsubscribe you from all text messages sent be Leon Medical Centers. You agree that following a request to unsubscribe from receiving texts, you will receive on (1) final message confirming that you will receive no further text messages from Leon Medical Centers.
  • Please notify us immediately if your mobile number changes. Leon Medical Centers is not liable for any communication or transmission of information by text which happens because you did not report that your mobile number changed. Password-protecting mobile device(s) and enabling encryption, if available, is recommended.
  • Text messages may include protected health information (PHI). Since text messaging is unencrypted, there is a risk that this PHI could be intercepted or viewed by third parties, including others who access your device. When you choose to receive text messages from us, you do so at your own risk.
  • The use and disclosure of PHI in text messaging may be governed by additional privacy notices, including Leon Medical Centers’ HIPAA Notice of Privacy Practices.
  • Leon Medical Centers does not sell patient information to third parties.
  • Leon Medical Centers will not be liable for losses or damages arising from (a) non-delivery, delayed delivery, or misdirected delivery of a text message; (b) inaccurate or incomplete content in a text message; or (c) use or reliance on the contents of any text message for any purposes.
  • These terms and conditions are governed exclusively by the laws of the State of Florida.
